{"id":29183,"date":"2023-09-14T15:42:50","date_gmt":"2023-09-14T14:42:50","guid":{"rendered":"https:\/\/ppr-antibioresistance.inserm.fr\/info-communication\/news\/antimicrobial-resistance-the-silent-pandemic\/"},"modified":"2025-06-26T13:40:38","modified_gmt":"2025-06-26T12:40:38","slug":"antimicrobial-resistance-the-silent-pandemic","status":"publish","type":"post","link":"https:\/\/ppr-antibioresistance.inserm.fr\/en\/editorials\/antimicrobial-resistance-the-silent-pandemic\/","title":{"rendered":"Antimicrobial Resistance: The Silent Pandemic"},"content":{"rendered":"\n<p>Antimicrobial resistance (AMR), a \u2018silent\u2019 or \u2018overlooked\u2019 pandemic,<sup> 1,2 <\/sup>is classed as a threat to global health and development by the World Health Organization (WHO). If left unchecked, it could cause 10 million deaths per year by 2050,<sup>3<\/sup> cost the global economy US$100tn,<sup>1<\/sup> and drive 28 million people into extreme poverty.<sup>4<\/sup> This cannot be overlooked, and we cannot be silent \u2014 the development of novel antimicrobials must be a public health priority.<\/p>\n\n<p>Decisive political leadership, guided by expert opinion, is vital to navigating public health crises. The COVID-19 pandemic has shown that coordinated action is possible on national and international scales. However, the focus on SARS-CoV-2 has led to a substantial decline in public awareness of AMR and its hazards. <\/p>\n\n<p>AMR disproportionately affects those in low- and middle-income countries (LMIC),<sup>3,5<\/sup> where the lack of means for effective prevention, diagnostics and treatment of infectious diseases is combined with over-the-counter access to antibiotics. In South Asia and Latin America where access to second-line antibiotics is good, the AMR burden is due to high levels of resistance.<sup>6<\/sup> Global collaborations such as the Quadripartite collaboration<sup>7<\/sup> and the Global Leaders Group<sup>8<\/sup> are promoting a One Health approach to increase global awareness and surveillance of AMR a on an international scale.<\/p>\n\n<div style=\"height:31px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n<p style=\"font-size:25px\"><strong>One Health \u2013 the problem and the cure<\/strong><\/p>\n\n<div style=\"height:29px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n<p>The One Health concept aims to sustainably balance and optimize the health of people, animals and ecosystems. In the context of AMR, it is a multidisciplinary and multi-sectorial approach to preventing emergent and resurgent infectious diseases.<\/p>\n\n<p>Potential routes to AMR development are complex and linked to all OneHealth sectors (Figure 1 &amp; <a href=\"https:\/\/eur-lex.europa.eu\/legal-content\/EN\/TXT\/PDF\/?uri=CELEX:32022R1255\">Publications Office (europa.eu)<\/a>), for example:<\/p>\n\n<ul>\n<li>La surutilisation et le mauvais usage des antimicrobiens dans les milieux m\u00e9dicaux et v\u00e9t\u00e9rinaires,<sup>9<\/sup><\/li>\n\n\n\n<li>L&#8217;utilisation non r\u00e9glement\u00e9e et non contr\u00f4l\u00e9e d&#8217;antimicrobiens en agriculture,<sup>10<\/sup> entra\u00eenant en particulier une r\u00e9sistance aux antifongiques,<sup>11<\/sup><\/li>\n\n\n\n<li>Les r\u00e9servoirs environnementaux d\u2019antibior\u00e9sistance (par exemple, les eaux us\u00e9es et les d\u00e9chets non trait\u00e9s qui atteignent les r\u00e9servoirs d\u2019eau).<sup>5<\/sup><\/li>\n<\/ul>\n\n<div style=\"height:38px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n<div class=\"wp-block-image\">\n<figure class=\"aligncenter size-full\"><a href=\"https:\/\/www.cdc.gov\/narms\/images\/antibiotic-resistance-spread-300px.jpg?_=61088?noicon\"><img fetchpriority=\"high\" decoding=\"async\" width=\"750\" height=\"662\" src=\"https:\/\/ppr-antibioresistance.inserm.fr\/wp-content\/uploads\/2023\/09\/2-how_antibiotic_resistance_spreads.jpg\" alt=\"\" class=\"wp-image-29131\" srcset=\"https:\/\/ppr-antibioresistance.inserm.fr\/wp-content\/uploads\/2023\/09\/2-how_antibiotic_resistance_spreads.jpg 750w, https:\/\/ppr-antibioresistance.inserm.fr\/wp-content\/uploads\/2023\/09\/2-how_antibiotic_resistance_spreads-300x265.jpg 300w\" sizes=\"(max-width: 750px) 100vw, 750px\" \/><\/a><figcaption class=\"wp-element-caption\"><strong>Figure 1: Examples of how antimicrobial resistance spreads (<a href=\"https:\/\/www.cdc.gov\/narms\/faq.html\" target=\"_blank\" rel=\"noreferrer noopener\">CDC; National Antimicrobial Resistance Monitoring System for Enteric Bacteria (NARMS)<\/a>)<\/strong><\/figcaption><\/figure><\/div>\n<div style=\"height:33px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n<p>The One Health approach embraces a variety of AMR-countering strategies both scientific and logistical:<\/p>\n\n<ul>\n<li>Le d\u00e9veloppement de nouveaux antimicrobiens, d&#8217;outils de diagnostic pr\u00e9cis et rapides, de vaccins et d&#8217;immunoth\u00e9rapies efficaces,<\/li>\n\n\n\n<li>Une compr\u00e9hension approfondie de l\u2019antibior\u00e9sistance au niveau mol\u00e9culaire, des interactions cible-m\u00e9dicament, de l&#8217;immunologie des maladies infectieuses et de la dysbiose suite \u00e0 un traitement antibiotique,<\/li>\n\n\n\n<li>L\u2019am\u00e9lioration des programmes de gestion des antimicrobiens et de pr\u00e9vention des infections, l\u2019efficacit\u00e9 des programmes actuels \u00e9tant insuffisante,<sup>12<\/sup><\/li>\n\n\n\n<li>Am\u00e9lioration de l&#8217;acc\u00e8s aux ressources existantes en soins de sant\u00e9, une meilleure gestion de la cha\u00eene d&#8217;approvisionnement d\u2019antimicrobiens et une r\u00e9duction de l&#8217;exposition environnementale aux r\u00e9sidus antibiotiques,<\/li>\n\n\n\n<li>Am\u00e9lioration de la surveillance en sant\u00e9 publique et de la formation du personnel de sant\u00e9 au niveau mondial.<\/li>\n<\/ul>\n\n<div style=\"height:34px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n<p style=\"font-size:25px\"><strong>The essential component: Funding<\/strong><\/p>\n\n<div style=\"height:41px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n<p>Successful innovation relies on resource mobilization, but at present, antimicrobial R &amp; D is fragmented, with a lack of collaboration between academia, industry and clinicians. CARB-X (<a href=\"https:\/\/carb-x.org\/\">https:\/\/carb-x.org<\/a>) and the AMR Action Fund (<a href=\"https:\/\/www.amractionfund.com\/\">https:\/\/www.amractionfund.com<\/a>) are good examples of essential funding sources for novel antimicrobial development. Further funding by both public and private investors needs to be highly visible to prove that antimicrobial R &amp; D is a global priority.<\/p>\n\n<p>Interruptions to R &amp; D funding can lead to abandonment of projects, new firms failing to grow into mature businesses, and researchers in both academia and industry moving on to more financially rewarding areas of research. The collapse of companies that successfully brought new antibiotics to market, yet still suffered bankruptcy (e.g., Melinta Therapeutics and Achaogen), contribute to the lack of confidence for investment in antimicrobial R &amp; D. <\/p>\n\n<p>In response, funding schemes have been launched in the UK<sup>13<\/sup> and the USA<sup>14<\/sup> to reward firms that launch new antimicrobials through an annual fixed fee, delinking financial reward from the volume of prescriptions. To encourage international pharmaceutical companies to re-invest in antimicrobial R&amp;D, similar schemes need to be offered globally.<\/p>\n\n<p>In addition to R&amp;D funding, investment in the AMR workforce is necessary. Central governments can establish early career fellowships in AMR, provide training in high risk antimicrobial therapeutic and prophylactic innovation, and establish centres for AMR research (national\/international) that would act as hubs for R&amp;D. <\/p>\n\n<div style=\"height:45px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n<p style=\"font-size:25px\"><strong>Conclusion<\/strong><\/p>\n\n<div style=\"height:30px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n<p>To tackle AMR globally will require an integrated, collaborative and multi-sector response to AMR, substantial and sustained funding along with decisive leadership and resolute support from governments, global leaders, academia, industry and the not-for-profit sector.
