le projet ANORUTI se propose d\u2019am\u00e9liorer la compr\u00e9hension de ces \u00e9checs <em>in vivo<\/em> et de fournir des solutions de diagnostic avec une meilleure pr\u00e9cision de pr\u00e9diction. L\u2019objectif est de d\u00e9velopper <em>in vitro<\/em> des plateformes exp\u00e9rimentales de bior\u00e9acteurs et un mod\u00e8le pharmacocin\u00e9tique \u00e0 deux compartiments\u00a0 \u00a0pour \u00e9valuer la croissance et les capacit\u00e9s de survie des bact\u00e9ries dans diff\u00e9rentes conditions afin d\u2019identifier des souches ayant des comportements diff\u00e9rents lorsqu\u2019elles sont analys\u00e9es dans des conditions standard de mesure de la CMI et dans des conditions exp\u00e9rimentales imitant des environnements plus r\u00e9alistes de l\u2019infection urinaire. Seront examin\u00e9s l\u2019impact du pH, de la disponibilit\u00e9 du fer et de la r\u00e9ponse immunitaire de l\u2019h\u00f4te sur la sensibilit\u00e9 aux antibiotiques, la tol\u00e9rance aux antibiotiques, la persistance et la formation d\u2019un biofilm ainsi que les conditions qui pourraient conduire \u00e0 la non r\u00e9ponse au traitement. Ces exp\u00e9riences seront conduites avec des antibiotiques\u00a0 utilis\u00e9s seuls ou en association pour soigner les infections urinaires et une collection importante et diversifi\u00e9e de souches cliniques et g\u00e9n\u00e9tiquement d\u00e9termin\u00e9es. Bri\u00e8vement les f\u00e8ces de 100 personnes, s\u00e9lectionn\u00e9es en fonction de leurs habitudes alimentaires, seront caract\u00e9ris\u00e9es par des approches de m\u00e9tag\u00e9nomique et introduites dans les miniguts en pr\u00e9sence d\u2019antibiotiques, de souches bact\u00e9riennes sensibles et r\u00e9sistances aux antibiotiques y compris de souches reportrices du stress ou de l\u2019\u00e9tat m\u00e9tabolique des bact\u00e9ries. La strat\u00e9gie est de mener une \u00e9tude \u00e9pid\u00e9miologique de 2 ans sur le transport intestinal des r\u00e9sistances aux c\u00e9phalosporines \u00e0 spectre \u00e9tendu (ESC-E) et aux carbap\u00e9n\u00e8mes (CP-E) chez 500 chiens et membres de la maison (HM) avant, pendant et apr\u00e8s l&#8217;hospitalisation des chiens. \u00e0 partir des donn\u00e9es recueillies, seront test\u00e9es des hypoth\u00e8ses sur les \u00e9v\u00e9nements de transfert des clones et plasmides au sein et entre les chiens et HM, sur les <em>loci<\/em> de transmission \u00e0 l&#8217;h\u00f4te, sur la persistance intestinale des r\u00e9sistances, et sur les voies d&#8217;\u00e9volution intra h\u00f4te de ces r\u00e9sistances. L\u2019ensemble sera valid\u00e9 sur des mod\u00e8les cellulaires et <em>in vivo<\/em> et compl\u00e9t\u00e9 par des analyses sur les relations chien\/humain d\u2019un point de vue sociologique. Partant de ce constat, le projet MicroFlu4AMR articul\u00e9 autour de 4 axes se propose\u00a0: d\u2019\u00e9tudier la diversit\u00e9 fonctionnelle (g\u00e8nes et voies m\u00e9taboliques) des communaut\u00e9s microbiennes du sol impliqu\u00e9es dans la production d\u2019antibiotiques et les r\u00e9sistomes correspondants, de d\u00e9velopper une meilleure compr\u00e9hension des processus \u00e9cologiques et \u00e9volutifs dans les communaut\u00e9s du sol impliqu\u00e9es dans la production d&#8217;antibiotiques, et par cons\u00e9quent, \u00e0 leurs r\u00e9sistomes associ\u00e9s, d\u2019\u00e9lucider le r\u00f4le et les m\u00e9canismes du transfert horizontal et vertical de g\u00e8nes dans l&#8217;\u00e9volution et la diffusion des antibiotiques et de la r\u00e9sistance aux antibiotiques, et de d\u00e9couvrir de nouvelles classes naturelles d\u2019antibiotiques qui r\u00e9duisent le risque d\u2019\u00e9mergence de r\u00e9sistomes adapt\u00e9s. \u00c0 cette fin, des \u00e9chantillons de sol, s\u00e9lectionn\u00e9s pour couvrir la plus grande diversit\u00e9 microbienne possible, seront analys\u00e9s en s\u2019appuyant sur des approches m\u00e9tag\u00e9nomiques cibl\u00e9es et globales et de criblage ph\u00e9notypique et g\u00e9notypique innovant \u00e0 tr\u00e8s haut d\u00e9bit <em>via<\/em> la plateforme microfluidique \u00e0 base de gouttelettes qui permet l\u2019analyse de microorganismes individuels coupl\u00e9e \u00e0 la production d\u2019antibiotiques d\u00e9tect\u00e9e \u00e0 l\u2019aide de bact\u00e9ries poss\u00e9dant des g\u00e8nes rapporteurs sp\u00e9cifiques. L\u2019ensemble permet un tri des gouttelettes activ\u00e9es par fluorescence \u00e0 des taux allant jusqu\u2019\u00e0 1\u00a0000\u00a0par seconde, et le criblage d\u2019environ 1 million de microorganismes par heure. En parall\u00e8le, un syst\u00e8me permettant le s\u00e9quen\u00e7age du g\u00e9nome entier de milliers de bact\u00e9ries \u2013cultivables et non cultivables\u2013 tri\u00e9es est en cours de mise au point et permettra d\u2019identifier directement des groupes de g\u00e8nes biosynth\u00e9tiques de nouveaux compos\u00e9s antibiotiques ou comprenant des m\u00e9canismes de r\u00e9sistance aux antibiotiques. Le projet Mustart, regroupant sept puissantes \u00e9quipes de recherche sp\u00e9cialis\u00e9es dans la tuberculose (dont deux en milieu hospitalier) ainsi que deux partenaires d\u2019autres domaines apportant des approches m\u00e9thodologiques nouvelles, ambitionne de relever cet enjeu par des actions de recherche concert\u00e9es et int\u00e9gr\u00e9es, allant du laboratoire au chevet du patient. Ainsi, des mod\u00e8les de criblage innovants seront partag\u00e9s pour enrichir la liste de compos\u00e9s potentiellement synergiques et diff\u00e9rentes combinaisons seront \u00e9valu\u00e9es \u00e0 l\u2019aide de mod\u00e8les d&#8217;infection <em>ex vivo<\/em> et <em>in vivo<\/em> afin d&#8217;identifier des sch\u00e9mas multidimensionnels optimis\u00e9s. Le consortium r\u00e9unit sept partenaires aux comp\u00e9tences compl\u00e9mentaires sur des disciplines vari\u00e9es\u00a0: microbiologistes cliniques et mol\u00e9culaires, biochimistes, structuralistes, v\u00e9t\u00e9rinaires, cliniciens, pharmacologues, \u00e9cologistes et experts pour le microbiome intestinal. Le projet s\u2019articule autour de six enjeux dont le spectre complet de l\u2019activit\u00e9 antimicrobienne de ces nouveaux m\u00e9dicaments anti-infectieux, leurs activit\u00e9s bact\u00e9ricides\/bact\u00e9riostatiques, leur synergie\/antagonisme avec les antibiotiques existants, leurs m\u00e9canismes d&#8217;action et leur(s) cible(s) mol\u00e9culaire(s) dans diff\u00e9rents milieux bact\u00e9riens (<em>Enterococcus faecium<\/em>, <em>Staphylococcus aureus<\/em>, <em>Escherichia coli<\/em> et <em>Pseudomonas aeruginosa<\/em>), leurs m\u00e9canismes de r\u00e9sistance et les co\u00fbts d&#8217;adaptation de la r\u00e9sistance, leurs param\u00e8tres PK\/PD et leur efficacit\u00e9 sur diff\u00e9rents mod\u00e8les animaux d\u2019infections. Afin d&#8217;obtenir une activit\u00e9 s\u00e9lective sur les agents pathog\u00e8nes, deux antibiotiques de la famille des b\u00eata-lactamines seront combin\u00e9s au sein d&#8217;une m\u00eame mol\u00e9cule pour obtenir des pro-m\u00e9dicaments inactifs. L\u2019hydrolyse de la premi\u00e8re b\u00eata-lactamine par la b\u00eata-lactamase produite par la bact\u00e9rie responsable de l\u2019infection lib\u00e9rera la deuxi\u00e8me b\u00eata-lactamine sous une forme active. Le consortium NASPEC r\u00e9unit\u00a0: i) deux \u00e9quipes de chimie qui \u00e9tudieront les b\u00eata-lactames existants et leurs voies de synth\u00e8se pour concevoir des pro-m\u00e9dicaments appropri\u00e9s en introduisant des liens clivables\u00a0; ii)\u00a0une \u00e9quipe de biochimie qui \u00e9valuera les compos\u00e9s synth\u00e9tiques pour leur activit\u00e9 antibact\u00e9rienne, leur mode d\u2019action et leur sp\u00e9cificit\u00e9 pour les bact\u00e9ries productrices de b\u00eata-lactamases\u00a0; iii) trois \u00e9quipes de biologie structurale sp\u00e9cialis\u00e9es dans la cristallographie et l\u2019analyse par RMN des b\u00eata-lactamases et des transpeptidases, ainsi qu\u2019une \u00e9quipe sp\u00e9cialis\u00e9e dans la mod\u00e9lisation des interactions entre m\u00e9dicaments et cibles, qui contribueront \u00e0 la conception des mol\u00e9cules. Les trois esp\u00e8ces cibles (<em>Staphylococcus<\/em><em> aureus, Staphylococcus epidermidis et Escherichia coli<\/em>) ont \u00e9t\u00e9 s\u00e9lectionn\u00e9s pour le d\u00e9veloppement de phages \u00e0 usage th\u00e9rapeutique en clinique humaine et pour mettre en place des essais cliniques. Au-del\u00e0 de l\u2019innovation scientifique, ce projet vise \u00e0 \u00e9tablir les bases de la premi\u00e8re plateforme fran\u00e7aise d\u2019isolement, de caract\u00e9risation, de purification, de production et d\u2019application clinique de th\u00e9rapies par les phages. Trois esp\u00e8ces \u00e0 Gram n\u00e9gatif souvent multir\u00e9sistantes ont \u00e9t\u00e9 s\u00e9lectionn\u00e9es : <em>Escherichia coli<\/em>, <em>Klebsiella pneumoniae<\/em> et <em>Pseudomonas aeruginosa<\/em>. Le projet vise \u00e0 \u00e9tablir des bases de donn\u00e9es g\u00e9nomiques et ph\u00e9notypiques \u00e0 partir de collections de souches cliniques afin de mieux analyser la relation g\u00e9notype-ph\u00e9notype, \u00e0 d\u00e9terminer l\u2019impact de la pression antibiotique sur l\u2019\u00e9volution des isolats en milieu hospitalier et les causes d\u2019\u00e9chec des traitements. TheraEPI ambitionne d\u2019inhiber chimiquement les modifications \u00e9pig\u00e9n\u00e9tiques produites par les bact\u00e9ries, dans le but de contrecarrer les avantages de la bact\u00e9rie, renforcer les d\u00e9fenses de l\u2019h\u00f4te et de minimiser l\u2019\u00e9mergence de la r\u00e9sistance bact\u00e9rienne. Le projet combine une panoplie de comp\u00e9tences en biologie et \u00e9pig\u00e9n\u00e9tique des infections, en prot\u00e9omique, en chimio-informatique et en chimie m\u00e9dicinale, dans la conception de m\u00e9dicaments. Sept agents pathog\u00e8nes \u2013<strong>\u00a0<\/strong>des genres <em>Chlamydia<\/em>, <em>Legionella<\/em>, <em>Listeria<\/em>, <em>Pseudomonas<\/em>, <em>Staphylococcus<\/em>, <em>Streptococcus<\/em> et <em>Theileria<\/em><strong>\u00a0\u2013<\/strong> retenus comme mod\u00e8les d\u2019infection seront utilis\u00e9s pour cribler des chimioth\u00e8ques sp\u00e9cifique et originaux ciblant les m\u00e9thyltransf\u00e9rases d\u2019histones ou les prot\u00e9ines qui reconnaissent les histones m\u00e9thyl\u00e9s, afin d\u2019identifier les compos\u00e9s chimiques qui inhibent la croissance microbienne et\/ou r\u00e9duisent la r\u00e9pression de la r\u00e9ponse immunitaire de l\u2019h\u00f4te. Les meilleurs candidats m\u00e9dicaments seront optimis\u00e9s en termes d\u2019efficacit\u00e9, de s\u00e9lectivit\u00e9 et de propri\u00e9t\u00e9s pharmacologiques par chimie m\u00e9dicinale. Les cibles des compos\u00e9s actifs seront identifi\u00e9es en utilisant de nouvelles approches de prot\u00e9omique. L\u2019activit\u00e9 des compos\u00e9s sera \u00e9valu\u00e9e dans des mod\u00e8les infectieux complexes, notamment dans des cellules humaines primaires et chez la souris pour obtenir la preuve de concept de leur potentiel comme nouveaux agent antibiotiques.
